Why lithium batteries fail

Lithium-ion cells store enormous energy in a small package. Manufacturing contamination, separator defects, poor cell matching, defective battery-management systems, counterfeit cells, and inadequate charging protections can push a cell into thermal runaway — a self-accelerating reaction that produces jets of flame and toxic gas in seconds. Victims often have no time to escape, which is why battery fires so often cause severe burns, smoke inhalation, and death — particularly when a device ignites overnight or near an exit.
